A Cottondale man who left DCH Regional Medical Center without being discharged may be a danger to himself, police said Tuesday afternoon.

Lt. Teena Richardson, a spokesperson for the Tuscaloosa Police Department, said 48-year-old Jerry Dewayne Maples was being committed to the hospital for treatment Tuesday when he decided to leave and walked out of the North Entrance around 10 a.m.

Richardson said Maples was wearing a blue shirt, jeans and a brown baseball cap and that he also has a tracheostomy tube.

Richardson asked anyone with information on the whereabouts of Jerry Maples to call the Tuscaloosa Police Department at 205-349-2121. She said Maples is a danger to himself only.
